1. Cancellation Policy We require at least 24 hours' notice for all cancellations. Failure to provide adequate notice or missing an appointment entirely will result in a late-cancellation fee.
2. Timely Arrival To keep our schedule running smoothly, please arrive on time. If you are more than 10 minutes late, your slot will be marked as "missed," and you will be required to book a new appointment for a later date.
3. Clinical Delays Medicine is unpredictable. Emergencies may arise that cause your doctor to run behind schedule. We appreciate your patience during these times. To help us stay on track, please clearly state the reason for your visit to the receptionist so we can allocate the correct amount of time.
4. Prescription Renewals Please have your pharmacist fax a refill request to 1-844-222-8894. Allow up to two weeks for processing. To ensure your health is monitored, do not wait until you are out of medication, as your doctor may require an in-person or phone review before renewing certain prescriptions.
5. Telephone Consultations In rare circumstances, your doctor may offer medical advice over the phone. Please understand that your physician is usually busy seeing patients in person during the day, so any phone calls must adhere to the doctor's schedule.
6. Investigatory Results Receptionists are not authorized to provide medical results (lab work, imaging, etc.) over the phone. You must schedule a follow-up appointment to review your results and discuss next steps with your doctor.
7. Appointment Focus & Duration To ensure high-quality care and maintain the schedule, please limit each visit to one health concern. If you have multiple issues, additional appointments must be scheduled.
8. Routine Physicals Full check-ups are recommended every 12 to 24 months. These appointments are for general screening. Please do not bring specific new ailments to a physical, as there will not be enough time to address them properly alongside the exam.
9. Medical Forms and Paperwork Doctors will not complete forms dropped off at the front desk. You must book an appointment to review the paperwork together. Note that insurance or government forms are not covered by OHIP and carry a fee (Determind by doctors). Completion may take up to four weeks.
10. Passport Documentation Due to time constraints, we suggest asking other authorized professionals (lawyers, ministers, etc.) to sign passport papers. If you require a physician's signature, you must have been a patient here for at least two years.
11. Non-Insured Services Not all medical services are covered by OHIP (e.g., driver’s physicals or insurance forms). A list of these uninsured services and their associated costs, based on OMA guidelines, is available at our front desk.
12. Conduct and Harassment We maintain a zero-tolerance policy regarding harassment. Any verbal, physical, or sexual abuse directed at our staff, nurses, or doctors—whether in person, by phone, or via email—will result in immediate discharge from the medical practice
